  2. Review and assessment — We review the client's needs, timeline, required services, and available care information and complete the appropriate assessment.
  3. Personalized care planning — We develop a plan around the client's functional, personal, and clinical needs.
  4. Care coordination and start of services — We coordinate staffing, scheduling, communication, and the transition into care at home.
